Perfusion Changes in Perforator-Based Propeller Flaps

چکیده
Abstract Background To cover soft tissue defects, the perforator-based propeller flap offers option to rotate healthy into complex wounds. By rotating flap, perforator is torqued. As a result, perfusion changes are possible. Methods A retrospective data analysis of patients was done, who received defects lower extremity as well peri- and postoperative monitoring with laser-Doppler-spectrophotometry system. Additionally, patient-specific were collected. Results Seven identified. Four experienced early complications, two epidermolysis distal areas, three wound healing disorders, one partial necrosis. Intraoperative showed decline blood flow after incision especially at site. In case there prolonged declines up first day. Conclusion Torqueing by can cause an impairment in inflow outflow. If prolonged, perfusion-associated complications The identification viable particularly important. addition, conservative mobilization necessary compensate for impaired adapting

Lower limb perforator propeller flaps - clinical applications.
BACKGROUND A great variety of flaps have been used in lower limb reconstructive surgery: random pattern flaps, axial flaps and free microsurgical transfers with many variants. After further research on the cutaneous blood supply and improvement of microsurgical techniques, perforator propeller flaps began to be studied and used. MATERIAL AND METHOD A propeller flap is defined as an insular fl...

into a short “handle”. This was a full-thickness skin bridge at the pivot point where the proximal flap was rotated 180 degrees downward towards the recipient site. The bridge acted to offer cutaneous and subcutaneous continuity and maintained an intact subdermal plexus to serve as additional channel for venous drainage [2]. After the flap was transposed to the knee, a vein graft was utilised t...

The reconstruction of loss of substance due to trauma or oncological excision may have relevant functional and aesthetic implications. We report our experience in twenty-one cases of propeller flaps for the treatment of loss of substance of the upper and lower limbs. The etiology of defect was tumor excision in nine cases, trauma in seven cases, surgical wound complications in four cases, and c...
متن کاملLong-Term Patency of Twisted Vascular Pedicles in Perforator-Based Propeller Flaps
Background Propeller flaps require torsion of the vascular pedicle of up to 180 degrees. Contrary to free flaps, where the relevance of an intact vascular pedicle has been documented, little is known regarding twisted pedicles of propeller flaps. As secondary surgeries requiring undermining of the flap are common in the extremities, knowledge regarding the necessity to protect the pedicle is re...
